If you would like to get more information or schedule an appointment, complete the secure inquiry form. **At the present time, Dr. Fuller is only accepting new couples wanting to begin with Couples Intensive Treatment.
Step 2 – Complete the Confidential Initial Session Form
Once you have an appointment, complete the initial session form prior to the appointment. Each person attending therapy needs to complete the confidential initial session form. If minors are attending, please complete one form for each child or adolescent. If couples therapy, each person will need to complete the initial session form.
Step 4- Read the information for new clients.
Please read over privacy practices, social media, and communication policies at your convenience in our information for new clients. Privacy practices are also posted in our offices to make it easy for our clients to access at any time.
Step 5 – Sign the consent for treatment and financial agreement.
You may print the signature pages and bring them with you to the first session or we will have them available to sign when you arrive.
If you would like to allow coordination of care with another provider (for example, a psychiatrist, primary care physician, etc.), complete this form to authorize release of information:
To find out more about your health care coverage call between 9 and 3 pm during the week and speak to Margarita at (832) 832-0870. She can find out what your coverage would be and get back to you to let you know the coverage within 3 business days. You will be notified in one to three business days with information about your out of network mental health coverage
